Important:
Your wig purchase may be covered by your medical insurance. Ask your
physician for a written prescripton. The prescription should read
"Hair Prosthesis due to (diagnosis)."
We will issue
a sales receipt to read "hair prosthesis prescribed by physician" as well
as the total amount of the purchase on your invoice. Submit all of
the above to your insurance carrier.

if interested, submit. In a matter of minutes, your order will be
processed. You will be taken back to form where you can choose your color
and put in your email address. Then submit form to us and we
will notify you and process your order in a timely manner. You may
also pay by money order, cashier's check or personal check.

States Post Office Priority Mail: Shipping/handling is $6.00.
If you would like insurance, please let us know. Add $1.10 cents
